ABSTRACT

Objective:To explore the status of kinesiophobia in patients with heart failure during discharge transition period, and analyze its changing trend and influencing factors.Methods:The189 patients with heart failure treated in Henan Provincial People′s Hospital from February 2020 to April 2021 were conveniently selected as the research objects. The general situation questionnaire, the Tampa Scale for Kinesiophobia Heart, the Self-efficacy for Exercise Scale and the Control Attitudes Scale-Revised were used to investigate the patients 2-3 days before leaving the hospital, 1 month and 4 months after discharge. Results:The score of patients′ kinesiophobia deteriorated slightly from 2-3 days before discharge (48.62 ± 11.26) to 1 month after discharge (49.03 ± 11.24), and decreased with the extension of the transition time to 4 months after discharge (47.86 ± 11.11). The overall trend of improvement was statistically significant ( F = 17.92, P<0.01). The score of perceived disease control also showed the same change pattern, 2-3 days before discharge (27.34 ± 7.40), 1 month after discharge (26.18 ± 7.39), 4 months after discharge (27.76 ± 7.38), the difference was statistically significant ( F = 38.41, P<0.01). The score of the exercise self-efficacy of 4 months after discharge (35.63 ± 11.51) was higher than (34.00 ± 11.88) of 2-3 days before discharge and (34.20 ± 11.69) of 1 month after discharge ( F = 12.33, P<0.01). The generalized estimation equation showed that exercise self-efficacy ( B = -0.255, P<0.01) and perceived disease control ( B = -0.439, P<0.01) were protective factors of kinesiophobia in patients with heart failure. Path analysis showed that perceived disease control could not only directly predict patients′ kinesiophobia ( β = -0.461, P<0.01), but also indirectly affect patients′ kinesiophobia by acting on exercise self-efficacy (mediating effect = -0.21, 95% CI -0.293 - -0.136). The mediating effect accounted for 31% of the total effect. Conclusions:The kinesiophobia, perceived disease control and exercise self-efficacy in patients with heart failure during discharge transition were dynamic. Perceived disease control could not only directly predict patients′ kinesiophobia, but also indirectly affect it by acting on exercise self-efficacy. Nursing staff can develop nursing interventions based on perceived disease control or exercise self-efficacy to reducethe level of kinesiophobia inheart failure patients.

ABSTRACT

Objective:To explore the influencing factors of drug application deviation in elderly diabetes mellitus patients during hospital-family transition period. Methods:A total of 278 elderly diabetes mellitus patients in Henan Provincial People's Hospital from March 2019 to March 2021 were selected as the study subjects. All patients were followed up by telephone 1 week after discharge. The drug deviation evaluation tool(MDT)was used to evaluate the drug application deviation in patients during the hospital-family transition period. They were divided into drug application deviation group and non-drug application deviation group.Sociodemographic and disease-related data and medication management data were compared between the two groups.Logistic regression analysis was used to analyze the influencing factors of drug application deviation in elderly patients with diabetes mellitus during hospital-family transition period. Results:Of the 278 elderly patients with diabetes, 162(58.27%)had at least one drug application deviation during hospital-family transition period. The family care index was lower in drug application deviation group than non-drug application deviation group( Z=6.578, P<0.001).As compared with non-drug application deviation group, drug application deviation group had the higher number of drugs at discharge, and had lower scores of Morisky Medication Adherence Scale with Eight-Item(MMAS-8), had lower scores of Summary of Diabetes Self Care Activities(SDSCA)and had lower scores of Self-efficacy for Appropriate Medication Use Scale(SEAMS), with statistically significant differences( Z=10.971, 6.077, t=5.947, 14.105, all P<0.001).Binary Logistic regression analysis and forest map showed that the more number of discharge medication was a risk factor for drug application deviation during hospital-family transition period in elderly patients with diabetes mellitus( OR=4.128, P<0.001); family care index, MMAS-8 score, SDSCA score and SEAMS score were its protective factors( OR=0.343, 0.523, 0.922, 0.568, all P<0.05). Conclusions:The incidence of drug application deviation during hospital-family transition period is higher in elderly patients with diabetes mellitus. The possible protective factors are high scores of family care index, MMAS-8, SDSCA and SEAMS, and the risk factor is large number of drugs ordered by discharged doctors. Therefore, targeted intervention measures can be implemented to reduce the occurrence of drug application deviation during hospital-family transition period.

ABSTRACT

In this study we evaluated the effects of the prepartum anionic diet on the electrolyte balance and calcemia of high producing dairy cows in the first days of lactation, and investigated the impact on the frequency of subclinical hypocalcemia (SCH). Sixty healthy Holstein cows, producing 30 kg of milk/day, handled in intensive system (compost barn), were distributed in groups (n=15) according to lactation order: first, second, third, and fourth to sixth. In the last three weeks before calving they received a diet with negative DCAD (-6mEq/100g DM) and high chloride content. After calving, they received a diet with positive DCAD (18mEq/100g DM). Urine pH was measured before calving. Serum Na+, Cl-, K+, and total Ca concentrations, and the strong ion difference (SID3) were determined in samples taken soon after calving (0h), 24, 48, 72 and 96h after. The frequencies of SCH were determined considering the critical value of 2.125mmol/L (8.5mg/dL). Two-way repeated measures ANOVA and chi-square test were used for comparisons. The cows eliminated acidic urine before calving. Na+, K+, Cl-, and SID3 values did not differ between groups. Na+ and K+ did not vary between days; Cl- was elevated at calving and decreased until 72h; and SID3 was reduced at calving and increased up to 48h. The Ca levels were reduced until 24h and increased up to 72h. Cows of third and fourth to sixth lactations presented lower values up to 24h. SCH was observed in almost half of the cows (43.3% to 55%) until 48h. The maintenance of hypocalcemia for three or more consecutive days occurred in 53.3% of third and fourth to sixth lactations cows. Ingestion of a high chloride prepartum anionic diet led to hyperchloremic acidosis and this imbalance was reversed on the second postpartum day. The induced effects on electrolyte and acid-base balances were not able to prevent the occurrence of SCH in the first days of lactation.(AU)

As vacas eliminavam urina ácida antes do parto.

ABSTRACT

In high yielding dairy cows, one of the major cause of lameness during transition period is the altered nutritional status of pregnant animal. Concentrations of essential nutrients get varied and often decrease around calving due to physiological factors and thus extra-supplementation is recommended in these animals. Blood measures are frequently used in assessment of the vital nutrients for predicting the risk of lameness during this period. The objective of this study was to investigate the nutritional status of healthy and lame Karan Fries (KF) cows and their association with lameness score during transition period. For this fifteen high-yielding crossbreed (KF) healthy cows and fifteen high yielding crossbreed (KF) lame cows were randomly selected from the Livestock Research Centre (LRC) of National Diary Research Institute, Karnal for pooled blood sample collection. Samples were collected 60d, 15d pre-partum, on the day of calving, and 60d, 15d post-partum. The results revealed that the mean plasma biotin, zinc (below critical level) and calcium (within range) levels were significantly lower (P<0.05) in lame KF cows while β-hydroxybutyrate (BHBA) concentrations was significantly higher, however no significant difference in plasma copper, glucose and non-esterified fatty acids (NEFA) concentrations was found in lame cows as compared to normal KF cows during this period. There was also significant (P<0.05) variation of blood biochemical profiles (glucose, NEFA and BHBA) and plasma biotin concentration among different lameness scores (1-5) whereas no relation of mineral profiles (calcium, zinc and copper) was seen with lameness score

ABSTRACT

ABSTRACT: The aim of this study was to confirm the possible consequences of prepartum lameness on subsequent lactation among Holstein cows. In this research, 27 multiparous cows, were monitored from the 30th to 63rd day, relative to calving. Thirty days prior to parturition, the animals were segregated into two groups based on the locomotion score (LS), where LS 1 is attributed to animals without claudication, LS 2 to those with suspected lameness and LS 3, 4 and 5 to those with mild, moderate and severe lameness, respectively; in the lame cows group (LC) (n=15), the animals displayed LS ≥ 3, whereas in the non-lame cows group (NLC) (n=12) they showed LS 1. Milk production, body condition score (BCS) and blood concentration levels of β-hydroxybutyrate (BHB), urea, phosphorus, calcium aspartate aminotransferase (AST), gamma glutamyl transferase (GGT) and total plasma proteins (PPT) were evaluated. Milk production (17.675 ± 0.31 L of milk/day) and blood calcium concentration (7.42 ± 0.12 mg/dL) were lower in the lame cows in comparison to those without lameness (22.27 ± 0.42 L of milk/day and 9.63 ± 0.13 mg/dL). Besides, the lame cows showed higher BCS loss during the early postpartum period. The metabolites AST, GGT, PPT, urea, BHB and phosphorus revealed no difference between the groups during the period evaluated. The lameness evident in cows during prepartum exerted major negative effects on the milk production and calcemia, making the LS evaluation during these period an important tool in the early diagnosis of losses for future lactation.

ABSTRACT

Clinical and metabolic evaluation is necessary for the monitoring of pregnant and lactating mares, as they reflect the health of the animal. The body condition of the mare is an indicator of reproductive efficiency. The study aimed to determine the possible variations in body and metabolic condition in Mangalarga Marchador mares during the transition period. Forty-eight mares distributed in two groups were used: Maintenance Group (MG), composed of non-pregnant and non-lactating mares, and Transition Group (TG), formed by pregnant mares and who after delivery became lactating. Analyzes were performed in the times T-60, T-30 and T-15 before delivery, first six hours (T0) after delivery and T15, T30 and T60 days after delivery. MG was evaluated only at one time (T-60). Body weight and fat-free mass differed (P<0.05) between the groups. The percentage of fat was lower in MG. Mares had a higher fat percentage in TG at T-60 and T-30 times. There was a difference (P<0.05) in the amount of cholesterol between MG and TG (T0, T15 and T30). Triglycerides were different between the groups. TG showed higher concentrations of non-esterified fatty acids (P<0.05). There was a higher amount of glucose in TG (delivery and lactation) when compared to mares in MG. Changes in body condition and metabolic constituents occurred in the animals resulting from physiological adaptations of the transition period. The energetic components are the most affected from the transition period, with intense fat mobilization to supply the body demands.(AU)

ABSTRACT

This study evaluates the metaphylactic of the subcutaneous administration of a trace minerals and vitamins A and E, on the oxidative stress and neutrophil function in Holsteins cows under the transition period. Sixty females were divided in two groups: group with trace minerals and vitamins (GMTV) (n= 30) and group control (GC) (n= 30). There was no significant difference between those groups; however, we find significant difference between treatment and day for Superoxide dismutase (SOD) values with higher activity of this enzyme in females GMTV on the weeks next to the parturition. Still, there was difference on day and, for interactions between treatment and day for TBARS, were females GMTV showed lower values of TBARS in all moments, except on day seven after the parturition. For leucocytes, there is a significant effect by day, interaction on treatment and day on neutrophils, and interaction treatment and day for neutrophil phagocytosis, were females GMTV showed lower values of leucocytes and neutrophils next to the parturition, and an increase of neutrophils phagocytosis. In summary, cows from the GMTV group had an improvement on the immune and oxidative systems, probably correlated with the administration of this supplement.(AU)

ABSTRACT

In face of the few reports found in national literature analyzing the potential influence of parturition number in serum proteinogram and biochemical profile in the peripartum period of high yielding dairy cows, the aim of the present study was to comparatively evaluate the dynamics of these serum constituents' concentrations in blood samples obtained from primiparous and multiparous Holstein cows, 60 and 30 days prepartum and in the day of parturition. Data were analyzed by repeated measures variance analysis (ANOVA) and differences between groups and moments were analyzed by Tukey's test. Results were considered significant when P<0.05. Parity influenced levels of total protein, albumin, globulins, magnesium, cholesterol, which were higher in multiparous cows, as well as concentrations of ceruloplasmin, total calcium, chloride and alkaline phosphatase activity, which were higher in primiparous cows. Parturition influenced serum concentrations of ceruloplasmin (+58%), transferrin (-25%), haptoglobin (+33%), total protein (-17%), globulins (-25%), immunoglobulin A (-43%), immunoglobulin G (-24%), total calcium (-12%), inorganic phosphorus (-10%), chloride (+5%), sodium (+4%), cholesterol (-23%), triglycerides (-38.6%), as well as activities of aspartate aminotransferase (+14%) and alkaline phosphatase (+28%). A decrease in serum levels of total calcium, inorganic phosphorus, cholesterol and triglycerides was more pronounced in multiparous than in primiparous cows. These results demonstrate that the interpretation of proteinogram and serum constituents should take into consideration lactation number and the moment of parturition as relevant factors in high yielding dairy cows in the transition period.(AU)

ABSTRACT

ABSTRACT

The aim of this study was to compare the effects of glycogenic supply or lipogenic diets on blood metabolites and hormones in primiparous dairy cows (n=40) during the transition period. The animals were randomly assigned to four treatments: control, calcium salts of polyunsaturated fatty acids (Megalac-E, 100g prepartum and 250g postpartum), roasted soybeans (400g prepartum and 800g postpartum) and propylene glycol (300ml pre- and postpartum). The supplements were provided individually. Blood samples were taken to determine plasma concentrations of insulin, glucose, non-esterified fatty acids (NEFA) and IGF-I. The glucose and insulin concentrations were higher in calcium salts of polyunsaturated fatty acids treatment. The highest concentration of NEFA was observed in the control group. The plasma concentrations of IGF-I were higher for the treatments with lipogenic supplements. Adding calcium salts of polyunsaturated fatty acids diet was able to ease the hormonal and metabolic changes of the transition period features.(AU)

ABSTRACT

This study aimed to identify some biochemical and hormonal indicators during late pregnancy and early lactation from healthy dairy cows crossbred compared with those who had some type of clinical disorder throughout the experiment. Therefore, we used 39 Crossbred dairy cows (Holstein x Gyr), divided into two groups, the first (G1) with 22 healthy animals and the second (G2) with 17 cows showed some disease (retained placenta, endometritis, mastitis, foot rot and maternal dystocia). The experiment took place from collections made in the period -60, -40, -20, -10 days before delivery, 0 (parturition day), and +10, +20, +40, +60 days postpartum. We analyzed energy metabolites (glucose, fructosamine, NEFA and β-hydroxybutyrate), hormone (insulin and cortisol), protein (total protein, albumin, globulin and urea) and minerals (CaT, P, Mg, K, Na and Cl). The variables studied were interpreted by analysis of variance at 5% probability. Analyzing the energetic profile, there was a greater mobilization in G2 during childbirth, through the lower fructosamine and glucose values, besides higher concentrations of Agnes and β-hydroxybutyrate. The behavior of the hormones insulin and cortisol was similar, noting only effect of time, whose higher levels occurred on the day of delivery. The protein profile revealed by total protein only time effect in their lowest values were recorded on the day of delivery, however, albumin G2 was lower than the G1 at all times, since the G2 globulin were higher than the group of otherwise healthy cows and urea showed higher concentrations in G1. With respect to the total mineral calcium, magnesium and chlorine showed lower levels from the initial period of the collections in G2. It follows that these metabolites studied early signal nutritional deficiency during late pregnancy, reflecting the transition period, and compromising the adjustment mechanism for cows, thereby increasing the risks to higher incidence of disease.(AU)

ABSTRACT

The study aimed to evaluate the effect of supplementation with protected choline on the energy profile, liver enzymes and reproduction in dairy cows in peripartum. Fifteen cows were divided into two groups: 8 received 80 grams of protected choline for 21 days pre-partum and 40 days postpartum, and 7 were considered control. Blood samples were collected on days 10, 20, 30, and 60 postpartum to evaluate the energy and hepatic profiles. After 60 days postpartum a gynecological examination of animals for evaluation of reproductive health was done. The supplementation with choline protected did not alter the beta-hydroxybutyrate levels (BHBA), non-esterified fatty acids (NEFA), fructosamine, like growth factor glucose (IGF-I), total oxidant status (TOS), aspartate aminotransferase (AST) and glutamiltrasferase range (GGT) postpartum. There was no difference in milk production. After 60 days postpartum, cows fed protected choline had fewer cases of endometritis that cows in the control group. The protected choline supplementation did not alter the biochemical profile and milk production, but reduced the number of endometritis cases in postpartum dairy cows.(AU)

ABSTRACT

The transition period is a time of great challenge for dairy cows, since most metabolic problems occur at this time and can compromise the entire lactation, resulting in a significant economic loss to commercial farms. This study evaluated the metabolic profile of dairy cows during the transition period. Twelve Holstein cows between three weeks pre-calving until three weeks postpartum was used in this study. They remained in a free-stall system, located in Inhaúma, Minas Gerais, during October to December 2012. The metabolic profile were composed by non-esterified fatty acids (NEFA), beta-hydroxybutyrate (BHBA), cholesterol (COLES), total protein (TP), albumin (ALB), calcium, phosphorus, magnesium as well as the activity serum enzymes aspartate transaminase (AST) and lactate dehydrogenase (LDH). There were differences in serum NEFA and BHBA between prepartum and postpartum (p<0.05). Observed a decrease in the concentration of COLES with the approaching birth with subsequent increase (p<0.05). Serum concentrations of minerals, PT and ALB did not differ (p>0.05). The enzymatic activity of AST and LDH were higher in the postpartum period (p<0.05). The evaluation of the metabolic profile is an important monitoring tool. Changes in the metabolic profile of cows during the transition period are motivated by energetic changing. Cows that were evaluated have a small risk of developing postpartum diseases related to the transition period.

ABSTRACT

In early lactation dairy cattle suffer metabolic alterations caused by negative energy balance, which predisposes to fatty liver and ketosis. The aim of this study was to evaluate the metabolic condition of high yielding dairy cows subjected to three treatments for preventing severe lipomobilization and ketosis in early lactation. Fifty four multiparous Holstein cows yielding >30 L/day were divided into four groups: control (CN= no treatment), glucose precursor (PG= propylene-glycol), hepatic protector (Mp= Mercepton®), and energy supplement with salts of linolenic and linoleic faty acids (Mg-E= Megalac-E®). Treatments were administrated randomly at moment of calving until 8 weeks postpartum. Blood samples were collected on days 1, 7, 14, 21, 28, 35, 42 and 49 postpartum. Body condition score (BCS) was evaluated at the same periods and milk yield was recorded at 2nd, 4th, 5th, 6th, 7th, and 8th weeks of lactation. Concentrations of non-esterified fatty acids (NEFA), albumin, AST, ß-hydroxybutyrate (BHBA), cholesterol, glucose, total protein, urea and triglycerides were analyzed in blood samples. Cut-off points for subclinical ketosis were defined when BHBA >1.4 mmol/L and NEFA >0.7 mmol/L. General occurrence of subclinical ketosis was 24 percent during the period. An ascendant curve of cholesterol and glucose was observed from the 1st to the 8th week of lactation, while any tendency was observed with BHBA and NEFA, although differences among treatments were detected (p<0.05). BCS decreased from a mean of 3.85 at 1st week to 2.53 at 8th week of lactation (p=0.001). Milk yield was higher in the Mg-E group compared with the other treatment groups (p<0.05) Compared with the CN group, the treatments with Mp and PG did not show significant differences in blood biochemistry and milk yield. Cows receiving PG and Mg-E showed higher values of BHBA and NEFA (P<0.05), indicating accentuated lipomobilization. Supplementation with Mg-E also resulted in significant higher concentrations of cholesterol, BHBA, urea, AST and lower values of glycemia. This performance may be explained by the highest milk yield observed with this treatment. Treatments with PG and Mp did not improve milk yield, compared with control cows, but did not show metabolic evidence of ketosis, fat mobilization or fatty liver. These results suggest that treatment with Mg-E improves milk production but induces a higher negative energy balance leading to moderated lipomobilization and ketone bodies production, increasing the risk of fatty liver.

ABSTRACT

In dairy herds, the interval between calving and pregnancy depend on many factors including preparation of the cow for calving, dietary management before and after calving. Factors such as hypocalcemia and body condition are determinant in the presentation of postpartum pathologies and ovarian activity resumption. Additionally, management of the dairy cattle should include fixed time artificial insemination protocols to increase the number of animals bred close to the voluntary waiting period and pregnancy rates. The following review will address topics related with cattle body condition and puerperal hypocalcemia that affect negatively postpartum reproductive performance and also will, address management alternatives to improve pregnancy rates y to reduce days open.

ABSTRACT

From an animal health-well being, welfare, and performance perspective, the postpartum period is comprised of an early window where acute health and cow survival is an issue with the development of septic metritis. Subsequently, sustained presence of endometritis is likely to be contributing to sub-fertility in lactating dairy cows with an overall herd pregnancy rate of approximately 16%. The challenge is to integrate on commercial dairies preventive medicine programs with reproductive management to improve herd fertility. Cows affected by periparturient disorders such as hypocalcemia, dystocia, and retained fetal membranes are more likely to contract uterine infections than cows that calve normally are. Thus, appropriate management of the transition period is critical in the prevention of uterine infections at the herd level. In addition, a postpartum program to monitor health to provide treatment to cows in the early stages of disease should be implemented.
